What is included in a Comprehensive PT evaluation and/or Health Coaching Plan?

Let’s take a look.

A Comprehensive Evaluation for Complex Symptoms

Many patients come to Wendy4Therapy after years of feeling dismissed, misunderstood, or unsure how their symptoms fit together. The new patient evaluation is designed to slow the process down, look at the whole person, and create a practical plan that makes sense for your body.

This evaluation is especially helpful for patients with complex pain, hypermobility, suspected EDS/HSD, POTS or dysautonomia symptoms, MCAS concerns, chronic fatigue, headaches, joint instability, pelvic symptoms, neurologic symptoms, or symptoms that seem to involve multiple body systems.

During the evaluation, Wendy reviews your history, symptoms, functional challenges, movement patterns, past therapy experiences, and personal goals. The goal is to help you understand what may be contributing to your pain, fatigue, instability, flares, and daily limitations, and to give you clear next steps for moving forward.

After the visit, you receive a detailed written report with findings, education, treatment recommendations, home exercise guidance, and suggested next steps. This report can also be shared with your physician, specialist, or local physical therapist to support coordinated care.

Your evaluation may include:

  • Review of your complete medical history

  • Review of current symptoms across body systems, including musculoskeletal, pain, skin, cardiovascular, GI, allergy/immunology, urogynecology, sleep, neurological, and psychosocial concerns

  • Discussion of symptoms such as sprains, strains, subluxations, muscle spasms, posture changes, stiffness, instability, headaches, jaw pain, sacroiliac pain, widespread pain, bruising, skin sensitivity, brain fog, nerve pain, clumsiness, proprioception issues, orthostatic intolerance, POTS-like symptoms, MCAS-like symptoms, constipation, bloating, food sensitivities, pelvic floor concerns, incontinence, anxiety, depression, fear of movement, and sleep disruption

  • Review of previous physical therapy, including what helped, what did not help, and what may have flared your current condition

  • Review of your current exercise routine, what you are able or unable to do, and your goals for movement and conditioning

  • Discussion of daily activities that are difficult, including work, school, household tasks, childcare, recreation, travel, sleep, and social activity

  • Review and discussion of your personal goals for therapy

  • Physical exam focused on anatomy, posture, functional movements, movement patterns, gait, balance, shoes, orthotics, and joint hypermobility

  • Beighton Score assessment, with optional hEDS worksheet review upon request and for an additional fee

  • Assessment of possible contributing factors such as joint instability, poor motor control, inefficient movement patterns, deconditioning, pain sensitivity, postural strain, autonomic dysfunction, or connective tissue-related concerns

  • A SUMMARY of evaluation findings and recommendations

  • A plan of care with the primary focus areas for ongoing physical therapy

  • Goals for ongoing therapy and self-management

  • Patient education tailored to your presentation, which may include EDS, collagen, hypermobility, dysautonomia, POTS, MCAS, pain, posture, pacing, body mechanics, ergonomics, sleep, nutrition, proprioception, braces, splints, taping, compression garments, manual therapy, and flare management

  • Education on how to turn the “wrong” muscles off and the “right” muscles on through neuromuscular re-education

  • Treatment recommendations that may include core stabilization, trunk/hip/neck strengthening, postural setting, diaphragmatic breathing, aerobic conditioning, biofeedback, muscle release techniques, soft tissue work, new movement patterns, task-specific training, and activities of daily living modifications

  • Guidance on managing long car rides, plane travel, sleep positions, flares, autonomic dysfunction, and communication with other medical professionals

  • Referrals or recommendations for further medical assessment when appropriate

  • Access to Wendy4Therapy educational resources, handouts, exercise guidance, and video materials

This evaluation is ideal for patients who want more than a quick exercise sheet. You will receive time, attention, education, and a plan designed to help you connect the dots, understand your body, and move forward with more confidence.
