Any reports from recent imaging or testing
Name and fax number of referring physician or primary care doctor so I can share the report
Comfortable clothing allowing me to see your arms and legs (consider shorts and tank top). I would also recommend a sweater as it tends to be chilly in my clinic
The shoes you wear most often AND the shoes you wear for exercise
Payment for services (Venmo, Zelle or Personal Check): $600 for initial evaluation and additional payment if additional services are requested

Consider bringing a caregiver or loved one as a lot of information will be shared and it can be helpful to have two sets of ears. You will not need to take notes as my report will detail everything we talked about.
Be prepared for a flight of stairs to my clinic located in the basement.
